Visit either of our Buckeye locations where we will take a new picture and issue a temporary driver’s license or ID paper copy until you receive your card in the mail within 10-15 business days.

A primary original document is required such as a birth certificate or current passport and your social security number. These items are required to verify who you are. 

We need your current passport or original birth certificate, your social security number, and two proofs of address. These two proof of address forms will need to show your current Arizona address and your full name.
